Sudden Hearing Loss: Why It's a Medical Emergency

If you wake up unable to hear from one ear, or notice a dramatic drop in hearing over a few hours, go to an emergency room, urgent care, or call an ENT physician immediately. Sudden hearing loss is a medical emergency with a narrow treatment window — every hour counts.
What Is Sudden Sensorineural Hearing Loss?
Sudden sensorineural hearing loss (SSHL) affects the inner ear and occurs rapidly — typically over hours or upon waking. Most cases affect only one ear, and about 90% of people notice the loss right away. You might experience a "pop" before losing hearing, or simply wake up to find one ear isn't working properly.
This is not the gradual hearing loss most people experience with age. It is an acute medical event that requires urgent evaluation, much like sudden vision loss or chest pain.
Recognizing the Warning Signs
Beyond reduced hearing, SSHL often presents with:
- Fullness or pressure in the affected ear
- Dizziness or vertigo
- Tinnitus — ringing, buzzing, or roaring sounds
- Difficulty understanding speech, even when sounds seem loud enough
Many people assume they have earwax buildup or a temporary blockage and wait to see if it clears. This delay can cost you your hearing permanently.
Why the 72-Hour Window Matters
The treatment window for SSHL is narrow. Research shows that starting treatment within the first 72 hours offers the best chance of recovery. The inner ear contains delicate hair cells that convert sound into electrical signals for the brain. When these cells are damaged or deprived of blood flow, they begin to die quickly.
Early intervention with corticosteroids — prescribed by a physician — can reduce inflammation, improve blood flow, and potentially preserve these cells before permanent damage sets in.
About 30-60% of cases see some spontaneous recovery, but waiting to find out if you're in that group means risking permanent hearing loss that may have been preventable with prompt treatment.
What Causes Sudden Hearing Loss?
In about 90% of cases, no specific cause is identified. Known causes include viral infections affecting the inner ear, circulatory problems, autoimmune disorders, head or acoustic trauma, certain medications, and rarely, tumors on the auditory nerve. Risk factors include diabetes, high blood pressure, smoking, and recent upper respiratory infections — but SSHL can affect anyone at any age.
Your First Step: Seek Medical Care Now
Go to an ENT physician, urgent care, or emergency room immediately. Do not wait. A physician will evaluate you, rule out other causes, and can prescribe corticosteroids — the primary treatment for SSHL. They may also order imaging like an MRI to check for underlying conditions such as acoustic neuromas.
Do not delay seeking care because you think you may be overreacting. Sudden hearing loss affects your safety, your ability to communicate, and your quality of life.
How We Can Help After Medical Treatment
Once you've received medical care, an audiologist plays an important role in what comes next. If hearing doesn't fully return, our office can help you understand your options and adapt effectively. At Port Jefferson Hearing, our approach to long-term management includes:
- Comprehensive hearing evaluation to document your current hearing status
- Hearing aid fitting with Real Ear Measurement to verify appropriate amplification for your specific loss
- CROS or BiCROS systems for single-sided deafness when one ear has significant permanent loss
- Auditory training with LACE AI Pro to improve speech understanding in noise
Earlier hearing aid fitting generally leads to better outcomes than waiting — the brain adapts more readily when amplification begins sooner after hearing loss stabilizes.
